Outpatient Pharmacist; Astoria Queens; Full Time; Days

Pharmacists will provide medication and related pharmaceutical care to inpatients and outpatients in collaboration with other health care providers to assure that drug therapy is safe, effective, efficient and cost-effective. The Pharmacist will serve as a role model by practicing and exhibiting professional and exemplary behaviors when working with patients, visitors and fellow co-workers.
1.Dispenses medications accurately and in a timely fashion
2.Resolves questions regarding formulary and non-formulary medications
3.Knowledge and follows all medication use Policies and procedures
4.Knows and adheres to hospital wide and departmental policies
5.Performs routine medication order verification accurately and in a timely fashion
6.Provides dosing and therapeutic recommendations that take patient age/kinetic variables into account based on literature
7.Accurately prepares CSP (Compounded Sterile Preparations) in accordance with federal, state, and local laws, rules, regulations and standards, including compounded sterile hazardous preparations
8.Maintains adequate medication supply in the Pharmacy area and informs supervisor of impending shortages
9.Intervenes and documents interventions to correct/improve pharmacotherapy
10.Reports adverse drug events and medication errors occurring in area serviced
11.Takes responsibility for the operation of the assigned Pharmacy area and activities of all supportive personnel
12.Maintains and demonstrates professional competency by attending workshops, conferences, and C.E. programs.
13.Provides drug information and counseling as needed to patients, families and other hospital staff
14.Participates in departmental and hospital wide quality improvement activities
15.Participates in Hospitals Annual Influenza Vaccination Program
16.Reviews patient record to assess the appropriateness and completeness of the following:
a)Medication dose, route, frequency, rate, indication for use and administration instructions
b)Age, sex, height, weight, pregnancy and lactation status, renal and hepatic functions
c)Reviews available lab data and microbiology results
d)Assesses medication regimen to identify potential therapeutic duplication, drug-drug interactions, drug-disease interactions and drug-food interactions
e)Assesses anti-infective therapy for appropriateness and duration
17.Responsible for and accurately maintains records of the inventory, purchase, storage, administration and dispensing of controlled substances while ensuring compliance with all hospital policy and procedures and applicable local, state, and federal laws, rules and regulations governing controlled substances
- BS or Pharm D from an accredited school of Pharmacy
- ASHP accredited residency or advanced degree preferred
- NYS Department of Education License and current registration
- NYS Certified Immunizer preferred
- 2 years inpatient hospital Pharmacy practice preferred
- Name: Pharmacist License and current registration Issuing Agency: NYS Department of Education
- NYS Certified Immunizer preferred
