Summary
IU Health Home Health provides our patients with comprehensive care delivered by a highly trained interdisciplinary team. The IU Health Home Health LPN will work closely with the RN case manager to provide skilled care in patients’ homes. The traditional, complementary, alternative, and holistic services that IU Health Home Health professionals offer add to our patients’ overall quality of life.
Home Health nurses will experience a team approach to serve patients while enjoying flexibility and autonomy to provide individualized care. Nurses provide one-on-one patient centered care utilizing critical thinking and clinical skills including (but not limited to) infusion therapy, central line care, catheter care/management, wound care, VAC management, ostomy care/management and patient education. Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.
- Requires graduation from a nationally accredited licensed practical nursing program.
- Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license.
- Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Successful completion of orientation required.
- 1-2 years of relevant experience preferred.
