About the role


The LPN, at the request of the Intake Department, provides nursing services to patients as ordered by the physician.  The On-Call Hospice LPN ‘s primary responsibility is to respond to hospice patient's and caregivers needs at the patient’s home

Hospice care may take place in a hospice institution or the patient’s home. Each hospice patient is cared for by a team of professionals who provide medical care, pain management, and emotional and spiritual support tailored to the patient’s wishes.

Primary Job Responsibilities

  • The On-Call Hospice LPN ‘s primary responsibility is to respond to hospice patient's and caregiver's needs at the patient’s home
  • Provides patient revisits and pronounces
  • Provides skilled nursing care focusing on pain and symptom management.
  • Provides interim guidance to patients and caregivers regarding diagnosis, medications, and treatments, to provide continuity of care and support for patients and caregivers.
  • Works closely with the Interdisciplinary Team
  • Documents with OASIS in a timely manner

Requirements

  • Graduate of an accredited LPN program
  • Current LPN license (PA)
  • Minimum of one year experience in a hospice homecare setting
  • Preference for knowledge of OASIS
  • Maintains clinical and professional competence
  • CPR certified
  • Driver's License and Car Insurance in good standing
  • Reliable vehicle
  • Possesses the ability to follow oral and written instructions
  • Possesses the ability to communicate effectively with all members of the care team
  • Possesses good organizational skills.
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
  • Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
