Clinical Pharmacist - Second Shift (12:30 PM – 9:00 PM)

About the role

All duties and responsibilities are to be performed in accordance with VITAS Healthcare Corporation’s policies, procedures, guidelines, contractual commitments and governmental regulations.

 

·        Manage clinical pharmacy programs according to policies and regulations

·        Review records of patients to determine the appropriateness and cost-effectiveness of medication therapy

·        Evaluate patient’s condition to ensure all issues are being treated

·        Identify untreated health problems and make clinical recommendations to clinicians

·        Develop effective medication plans that minimize the risk of adverse side-effects

·        Consult on dosages, conversion strategies, palliative benefit analysis, etc.

·        Advise on the correct administration of drugs

·        Assess the results of pharmaceutical treatments

·        Collaborate with healthcare professionals to ensure optimal patient care

·        Keep accurate documentation of medication plans and recommendation adherence

QUALIFICATIONS

 

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

 

Experience:

·        Minimum 3 years of training and practical experience as a hospice/palliative care clinical pharmacist

·        Excellent understanding of pharmaceutical therapy and direct patient care

·        Extensive knowledge of drug administration techniques and alternative routes of administration

·        Aptitude in problem-solving and decision-making

·        Background in cost-effectiveness evaluation is a plus

·        Ability to work on various assignments simultaneously

·        Team player with strong interpersonal skills within all levels of the organization  

·        Ability to navigate within automated systems and proficiency in Microsoft Teams, Outlook, Word, Teams and Excel

 

Education:

Professional degree in Pharmacy from an accredited college or university or the international equivalent required.

 

Certification & Licensure:

Valid, current pharmacy state licensure required.
