The BH Care Coach provides prevention education, supports transitions between care settings, and coordinates post-discharge care for members transitioning from emergency departments or correctional facilities in support of the Virginia Medicaid contract. For pregnant and parenting members, the program includes the development of a Plan of Safe Care, ensuring that both the mother and child receive comprehensive support to address substance use and its associated risks. The BH Care Coach provides access to a full continuum of evidence-based BH and recovery treatment services.

  • Ensure a comprehensive BH Care Management model, to improve health outcomes, reducing barriers to treatment, and ensuring that members with behavioral health diagnosis receive the necessary care and support to achieve long-term recovery.
  • Provide overall care coordination for members with BH disorders to monitor treatment progress and promote recovery and relapse prevention skills
  • Knowledgeable about the telehealth delivery system in Virginia and must refer members in rural and other hard to access areas to these systems in order to receive an assessment and needed services for treatment and recovery.
  • Utilize high-quality, evidence-based use services through personalized care coordination, crisis intervention, peer support, and strong collaboration with medical, and behavioral health providers.
  • Provide comprehensive, integrated support to members experiencing behavioral health issues and possible co-occurring conditions, including children, adolescents, adults, and justice-involved members. 
  • Coordinate behavioral health and medical services, ensuring appropriate provider engagement and adherence to treatment plans.
  • Improve member’s health literacy about telehealth  providers while simultaneously addressing health related social needs to positively impact member’s healthcare outcomes and well-being.
  • Collaborate with internal departments, providers, and community-based organizations to link to appropriate services and create a seamless, culturally competent care experience that respects the members’ preferences and needs.
  • Follow processes, and procedures to ensure compliance with regulatory requirements by the Virginia Department of Medical Assistance Services (DMAS), Center for Medicare and Medicaid Services (CMS) and the National Committee on Quality Assurance (NCQA).

Required Qualifications

  • Bachelor’s degree in a health or human services field, QMHP, MSW, or BSW.
  • Minimum of 2 years of experience working directly with individuals with behavioral health diagnoses.
  • Ability to implement structured care management for transitions of care and provide overall care coordination.
  • Must reside in the commonwealth of Virginia. Central Region of Virginia preferred (Richmond and surrounding areas.)
  • Exceptional oral and written communication and interpersonal skills with the ability to quickly build rapport.
  • Ability to work with minimal supervision within the role and scope.
  • Ability to use a variety of electronic information applications/software programs including electronic medical records.
  • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel.

Preferred Qualifications

  • LMHP, LBSW, LMSW or RN/LPN.
  • Case Management Certification (CCM).
  • Experience working with Medicare, Medicaid and dual-eligible populations.
  • Knowledge of community health and social service agencies and additional community resources including but not limited to social determinants of health and the criminal justice system.
  • Experience with health promotion, coaching and wellness.
  • Previous managed care experience.
  • Bilingual preferred (Spanish, Arabic, Vietnamese or other).

  • Workstyle: This is a remote position
  • Workdays and Hours: Monday – Friday; 8:00am – 5:00pm Eastern Standard Time (EST).
