Contract:                  Fixed term up to 1 year.

Hours:                       37 

Salary:                       £29,777 – £40,221 per annum plus £3k market supplement.

Closing date:           25 July 2024 @ 23.59

We are looking for an experienced Care Manager/Social Worker to work with us in our Adult Social Care teams who are leading the way in health and social care integration.  This is a great opportunity to join a nationally recognised and committed team, supporting service users and their carers every step of the way.  If you are looking for a chance to work in a service where you can add real value – this could be the role for you!

About you…

Educated to A Level/NVQ3 standard or equivalent or Educated to NVQ4 or equivalent. For qualified Social Worker Dip. S.W. or equivalent.

  • Social Work England Registered
  • Previous experience in an Adult services setting
  • Understanding of Community Care legislation and multi- disciplinary working
  • Able to complete social care assessments
  • Manage a caseload

About the role…

As the Social Worker or Care Manager, Contact and Response Team, you will be responsible for the following:

  • Facilitate efficient assessment of Rutland Adult residents
  • Ensure safe, timely and effective crisis intervention and planned work through the completion and coordination of assessments
  • Commission services and support in response to assessed need
  • To promote and contribute to the safety and well-being of all people in the community through the implementation of the Rutland safeguarding adults’ practices and procedures and multi-agency risk management.
  • Maintain accurate and timely assessment and case records. Monitor and review support plans to ensure ongoing effectiveness
  • To work in an integrated way with partners including health and colleagues across LLR and locally

In addition, we can offer manageable caseloads, with reflective peer practice, CPD sessions, career development and regular 1 to 1 supervision.

This is a fixed term contract up to 1 year to cover for maternity leave.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS and Social work England registration are paid by us.
