Qualifications:

 

 Education, Training and Experience:

Must have high school diploma/G.E.D. and have and have acquired or meet the qualifications for certification as a Certified Recovery Specialist

 

Experience:

At least 12 months of related experience.

  This individual is responsible for providing case management services and will be trained to conduct Recovery Management Checkups (RMCs). The CHW will utilize the SAMHSA approved Relapse Prevention Therapy curriculum and Wrap Around/PBRSS that includes but is not limited to transportation; employment services/job training; community case ma,1agement to provide coordination and linkages with other services; referrals; life skills training and education on SUD/COD. The CHW is trained to lead New Client Orientations and psychoeducation groups designed to improve access to and retention in MAT and facilitate long-term recovery.   Your responsibilities in this role may include:   -Treat all clients and staff with dignity and respect -Maintain professional ethical boundaries and the confidentiality of all individuals supported -Assist individuals in recovery with articulating their personal goal(s) for recovery and developing their individualized recovery/ aftercare plan -Identify and support linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.) that support the individual's goals and interests -Identify barriers to full participation in community resources and work with individuals to develop strategies to overcome those barriers -Liaison with
