We're searching for a dedicated Float Registered Nurse to assist in covering patient care for various care teams. As an RN with us, you'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients through telemedicine. Responsibilities include managing patient care through regular assessments and updates to care plans, overseeing medication management, coordinating care with the interdisciplinary team, and facilitating advanced care planning. What you’ll do

  1. Patient Care 
    1. Triage responsibilities
      • Conduct thorough assessments via phone to evaluate patient symptoms, conditions, and concerns
      • Determine urgency of care and provide accurate and timely medical advice/care recommendations based on severity of condition(s)
      • Provide appropriate patient follow-up and education
    2. Enrollment responsibilities
      • Conduct telehealth visits assigned to their schedule, assessing if the patient meets criteria for palliative care services and referring to the appropriate program under direction of the Director of Palliative Care and Registered Nurse managers
      • Assess and enroll patients and as needed and complete initial referral follow-ups
      • Complete initial certification of illness (CTI) for value based programs, if applicable
      • Complete initial care plan upon enrollment and ensure ongoing assessments, updating the care plan as appropriate
    3. General responsibilities 
      • Provide patient care within scope of practice and per provider order
      • Develop and update care plans based on ongoing assessments
      • Manage patient medications, including implementing orders, maintaining current medication lists, assisting with medication refills, and ensuring adequate supply
      • Facilitate requisition of medical supplies and durable medical equipment
      • Complete care coordination activities, ensuring timely documentation updates and communication with primary care providers and other physicians
