Job Summary:
Join the University of Colorado School of Medicine as we launch a new Endocrinology clinic serving the South Region. This is a unique opportunity to help build a program from the ground up and make a meaningful impact on the lives of patients living with diabetes. As a Certified Diabetes Care and Education Specialist (CDCES), you will play a vital role in empowering patients to successfully manage their condition through education, coaching, and individualized care plans. You will work alongside a collaborative team of endocrinologists, nurses, and clinical staff in a patient-centered environment focused on improving outcomes and delivering exceptional care.
Key Responsibilities:
Provides comprehensive diabetes self-management education and training to patients with diabetes and their families using curriculum approved by the American Diabetes Association.
Collaborates with the multidisciplinary diabetes care team to develop, implement, and evaluate individualized plans of care.
Reinforces diabetes education and evidence-based practices with nursing staff and other clinical support personnel to promote consistent, high-quality patient care.
Develops, maintains, and distributes diabetes education materials for patients, families, and the community.
Demonstrates strong critical thinking, sound clinical judgment, and the ability to work independently with minimal supervision. Thrives in a fast-paced environment, prioritizes competing demands effectively, and takes appropriate action to support safe, high-quality patient care.
Work Location:
Onsite – this role is expected to work onsite and is located in Highlands Ranch, CO.
