About the role
Overview
At Houston Methodist, the Advanced Practice Provider (APP) Post Graduate Fellowship position is a part of a twelve-month program with rotations in assigned specialty areas. This position will be given the opportunity to learn and improve assessment and diagnostic skills, as well as treatment implementation through medication management, procedures, and other therapies in the context of specialty care. The APP Provider Fellow position will be responsible for providing direct patient care in a variety of team-based settings with the goal to transition to independent practice upon completion of their fellowship.Houston Methodist Standard

Responsibilities
PEOPLE ESSENTIAL FUNCTIONS- Provides patient care in assigned department by assessing and diagnosing clinical conditions, developing treatment plans and documenting appropriately.
- Uses therapeutic communication to establish a relationship with patients, families and interprofessional health care team to collaborate on the plan of care.
- Communicates in an active, positive and effective manner to all interprofessional team members and reports pertinent patient care and family data in a comprehensive and unbiased manner, listens and responds to the ideas of others.
- Provide direct patient care services, including performing physical assessments, diagnosing and managing acute and chronic conditions, prescribing medications, ordering and interpreting diagnostic tests, and developing and implementing treatment plans in their respective specialty area(s).
- Advocates on patient/family's behalf to identify and resolve clinical and ethical concerns, utilizing appropriate resources.
- Obtains superior patient satisfaction scores via standard patient satisfaction survey tool.
- Performs essential functions of assessment, diagnosing, treatment, and management of patients in assigned setting, including accurate and timely documentation, intake assessments, treatment planning and discharge planning, as appropriate.
- Collaborates with physicians, nurses, and other members of the healthcare team to ensure comprehensive and coordinated patient care.
- Actively participates in interprofessional team meetings, case discussions, and care planning. Engages in team based interdisciplinary care.
- Self-motivated to independently manage time effectively and prioritize daily tasks.
- Maintains awareness of their direct and indirect financial impact on the organization.
- Keeps up-to-date with current evidence-based practice guidelines and professional standards.
- Seeks guidance and feedback from preceptors and healthcare professionals to enhance clinical skills and knowledge. Attends educational sessions, labs, and lectures to further develop skills in their respective specialty.
