Personalized Recovery Oriented Services (PROS) program reflects the latest evidence-based practice in psychiatric rehabilitation. PROS integrate treatment, skills training, vocational rehabilitation, and support to facilitate a participant’s recovery. In a safe and caring environment, participants are directly involved in all aspects of their services, from goal setting to individual recovery plan development and implementation. There is a strong focus on participant and family participation as well as evaluation of outcomes. Participants are encouraged to engage in community activities, adult education classes, and other opportunities that strengthen their supports and overall quality of life. PROS also place an emphasis on helping participants find meaningful employment, which is strongly linked to positive mental health.
Position Summary: The Nurse provides injections, preauthorization’s, health assessments, medical services and group facilitation services the PROS Program and provides relief nursing coverage to the OMH Adult Clinic Program when necessary. The nurse works closely with agencies prescribers and the PROS staff team.
Essential Duties and Responsibilities:
- Provide group and individual wellness education and perform general health assessments to participants in the PROS
- Complete necessary documentation, including health assessments and progress notes, according to OMH, Medicaid and agency standards
- Complete pre-admission and intake authorizations
- Intervene in crisis situations
- Collaborate with service providers and other stakeholders as appropriate
- Review blood work and lab reports, and inform treating psychiatrists and nurse practitioners of emergent issues. Follow up as appropriate
- Assist psychiatrists and nurse practitioners in providing timely responses to requests for prescription refills
- Perform injections of psychotropic medications as needed
- Inventory control of all medications, including medication samples and injection medications
- Exercise good professional and ethical judgment
- Maintain appropriate professional boundaries
- Demonstrate reliability and conscientiousness
- Strong communication and team building skills
- Good organization and time management skills
- Perform other duties as assigned
Requirements
Physical Requirements:
- Able to sit for long periods of time
- Occasional lifting of 20 lbs.
Minimum Qualifications:
- Registered Nurse (RN) certification required, with current licensure in New York State
- Strong verbal and written communication skills
- Demonstrated ability to work as part of a multi-disciplinary team
Preferred Qualifications:
- Experience providing nursing care to adults with severe and persistent mental illnesses preferred
- Experience providing group and individual medication education is strongly preferred
