Job Description:
The Registered Nurse (RN) is a licensed professional caregiver who assumes responsibility and accountability for assessing, planning, implementation and evaluating care of patients. The staff RN utilizes the nursing process by use of technology, therapeutic intervention, and coordination of care with other health team members.The Complex IBD program at Primary Children’s Hospital providers coordinated care for children, teens, and young adults with inflammatory bowel diseases (IBD) like Crohn’s Disease and ulcerative colitis. We are seeking a highly motivated Registered Nurse (RN) who will be responsible for the coordination of care for the IBD patient. The RN is a key member of the multidisciplinary team who provides individual patient education, care management, and coordination of services across the continuum of care. This role is responsible for the clinic aspects of the patient including, but not limited to education, patient care coordination, research, data management, and program development.
Job duties:- Coordinate medical, surgical, psychosocial, and nutrition evaluation and on-going care needs of Colorectal Center: Complex IBD patients
- Function as part of the multidisciplinary team; communicate patient needs to Colorectal Center team members
- Provide pediatric age-specific care and interventions
- Assess, plan, and advocate for the patient and families in coordination of care along the continuum with the Colorectal Center team, support personnel, community physicians, and outside agencies
- Provide accurate and timely documentation of care
- Demonstrate ability to independently make complex care decisions within established protocols
- Use critical thinking skills through the process, and involve the patient, family, physicians and the multidisciplinary team to establish and update the plan of care appropriately
- Participate in quality monitoring and process improvement activities and programs
- Participate in on-going education specific to IBD through attendance at professional meetings, webinars, and literature review
- Serve as a pathfinder and clinical expert by participating in, coordinating, developing, and implementing Colorectal Center program initiatives while monitoring and ensuring effective outcomes
- Evaluate patient response to interventions through review of achievement of goals, clinical outcomes, patient and family satisfaction
- Revise plan of care as indicated, and reassess changes as appropriate
- Promote nursing profession and participate in development of others Integrate legal and ethical standards into practice: comply with regulatory standards, practice within scope of licensure, provide accurate & timely documentation, and understand legal implications of care delivery
Scope
Staff RNs are pivotal members of the care delivery team headed by the physician and have specialized knowledge, skills and ability pertinent to ambulatory care patient needs. Scope of performance includes prioritization and triage of care delivery, implementing established standing orders and protocols, and performing procedures and therapies consistent with training and licensure. This position requires specialized knowledge and increased accountability, specific to high acuity and/or specialized patient populations (i.e. Instacares, Obstetrics, Infusion services, Sedation monitoring, etc).

Planning - Uses assessment information and critical thinking skills to collaboratively develop individualized plan of care; actively seeks patient and family involvement to develop plan of care; collaboratively plans and prepares patients for further support; prepares patients for further support needs once the current episode of care outcomes are met (i.e., teaching, referrals, and follow-up).
Implementation - Directs the interdisciplinary care team via delegation, coordination, and collaboration.
Evaluation - Evaluates patient response to interventions through review of achievement of goals, clinical outcomes, patient and family satisfaction; revises plan of care as indicated, and reassesses changes as appropriate.
