POSITION SUMMARY:

The Manager of Care Management has well developed skills in the assessment and acute care management of patients and families. The scope of practice includes assessment and management of high-risk, high-cost patients, multidisciplinary team leadership, program development, clinical outcomes measurement, clinical and fiscal variance analysis. Offers clinical consultation to all staff and provides case management services within the area of responsibility for a select number of patients who meet high-risk screen indicators.

The Manager of Care Management has the authority and responsibility for providing leadership and supervision to utilization management staff, care managers, care coordinators and social workers for both inpatient and outpatient services. Participates in continuous quality improvement activities and educational experiences in support of departmental and divisional nursing philosophy and objectives, as well as Health System initiatives. Plans and participates in community outreach activities.

EDUCATION/TRAINING:

  • Bachelor’s degree in Nursing required.

LICENSURE/CERTIFICATION:

  • Current licensure in the State of Illinois as an RN.  
  • Must obtain certification in specialty within 2 year of hire. Acceptable specialty certifications are Certified Case Manager (CCM) or Accredited Case Manager (ACM).

MAJOR RESPONSIBILITIES:

  • Oversees case management staff and ongoing development of this function throughout the System. In conjunction with physicians, nursing staff and other health care professionals works to improve the quality of patient care and ensure the efficient and cost effective movement of patients through the health care continuum.
  • In conjunction with Director formulates department goals, objectives, standards, policy and procedures for quality and patient safety efforts and initiatives throughout the system. Develops and manages operating and capital budgets ensuring positive financial outcomes.
  • Develops and implements annual performance goals that reflect the organization’s strategic plan and values and assures compliance
