PRINCIPAL RESPONSIBLITIES:
- The Employee is responsible for specific duties as assigned.
- Directly work with homeless populations
- Work directly with Family Service Association to facilitate case management in the homeless coalition program
- Identify community resources that support incarcerated individualism successful re-entry into the community.
- Partner with community resources in linking client to housing, treatment and social services that promote recovery(when needed) and lead to functioning in the community to reduce homeless, overdose and recidivism.
- Serve as a role model and mentor for clients and their families.
- Encourage client voice and choice during development and implementation of plans.
- Represent the Health Dept/FSA positively in the community, in meetings, with other agencies, at all times.
- Empower a client to have the confidence to be self-advocates.
- Help clients enhance relationships with community partners.
- Display excellent communication skills.
- Recommended project performance improvement, as needed.
- Assigned duties are completed within specified time frame.
- Supervisor review of Employee's documentation of assigned duties.
- Other duties as assigned.
Minimum Requirements:
- HS or GED
- Peer Support Specialist Certification with two (2) years in active recovery
Physical Requirement:
- Current TB skin test indicating no evidence of communicable disease.
Additional Requirements:
- Must possess a valid driver's license.
- Background checks conducted prior to hire.
