Damar is looking for a talented and motivated Community Based Case Manager Supervisor to join our team. The Case Manager Supervisor maintains a continuum of professional and administrative responsibilities related to the oversight and supervision of case managers and their respective cases. The Case Manager Supervisor oversees the interventions and case management services provided for every child and family served in our Community-Based program. This includes direct oversight of electronic and paper records, reports and documentation completed timely in our EMR, as well as processes and systems established to maintain the organization’s full compliance with regulatory standards of care. The Case Manager Supervisor may also facilitate onboarding of new providers as well as ongoing training and support to ensure providers are properly trained and up to date with DCS and stakeholder requirements.

Remarkable way you will make a difference...

  • Onboard all new providers and provider ongoing training as needed per DCS Service Standards.
  • Creates and maintains training modules for providers.
  • Ensures updated and thorough documentation is completed for each assigned independent contractor and client. Documentation is expected to be accurate and legible.
  • Supervises home based caseworkers and ensure their documentation is complete and submitted appropriately. approx. 20-25 bachelor level providers.
  • Hold monthly supervision with each provider.
  • Maintain a small caseload.
  • Review and approve monthly reports submitted by providers.
  • Collaborates with and supports team members in caseload assignments, scheduling service coverage and discharge
  • Provide intake assessment coverage for Family Preservation cases when needed.
  • Completes required tasks to ensure assigned department meets quality improvement, evaluation and reporting requirements.
  • Will be required to keep a small home-based caseload.

What you need to be considered...

  • Bachelor or Master's degree in Social Work, Psychology, Mental Health Counseling, or other closely related field. Master’s degree in a human services field preferred.
  • 3 years supervising/managing others required.
  • 5 years of experience with child welfare.
  • DCS experience a plus.
  • Licensed in one of the following preferred - LCSW, CSAYC, LSW, LMHC, LMFT, or LCAC.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
  • Successful completion of pre-employment drug screen and physical.
  • Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Checks.

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial trans
