Humana Care Support strives to assist in meeting members' needs and requirements so they can achieve and/or maintain an optimal wellness state.

The Care Management Support Professional contributes to administration of care management in coordination with the RNs, Social Workers and non-clinical associates on this team.

Responsibilities include:

• Using knowledge of social determinants of health (SDOH) and community resources to identify, research, document and coordinate services and resources to support member health and well-being.
• Serve as a key knowledge source for community services and information for member and other Humana associates.
• Providing education and coordination to meet SODH needs of members and their families including transportation, food, housing and other needs.
• Work collaboratively with other Humana associates as a member of Humana’s Care Management Support team.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences. 
• May complete assessments as warranted.

Required Qualifications

• MSW from a CSWE accredited school
• 2 years of Microsoft technical experience in Word, Excel, Outlook and an ability to troubleshoot and resolve general technical difficulties.
• 2+ years of experience supporting members social determinants of health (SDOH) needs
• Ability to learn new systems and work in multiple platforms.
• 2 years' experience working with individuals navigating community and/or healthcare resources.
• Experience working with people who have social determinants of health challenges.
• Exceptional communication & interpersonal skills with ability to build rapport with internal and external members and stakeholders over the telephone.
• Strong written and oral communication skills.
• Ability to multi-task and work in a fast-paced environment
• Demonstrated understanding and respect of all cultures and demographic diversity.

Preferred Qualifications

• Special Needs Plan (SNP) in Medicare and/or Medicaid
• Medical Social Work experience
• Experience working with members with medical and behavioral health co-morbidities.
• Experience advocating for managed care members at all levels of care.
• Non-clinical Case Management experience
• Call Center Experience
• Experience working remotely (WAH)

Work at Home Requirements

• Must have a separate room with a locked door that can be used as a home office to ensure you absolute and continuous privacy while you work.
• Must have accessibility to hardwired high speed internet with minimum speeds of 25 MBS Downstream/ 10 MBS Up-Stream.
• A hard-wired connection is required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

 

$45,400 - $61,300 per year
