We are seeking a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our Recuperative Care Program (RCP) Team at the 40 Prado Homeless Services Center. As an RCP Nurse, you will play a crucial role in monitoring the safety and stability of our participants. You will coordinate services with the RCP Case Manager, staff, and other professionals, including home health and on-site Community Health Center (CHC) medical staff, to ensure continuity of care and a smooth transition back into the community. Your work will directly support CAPSLO’s mission of helping individuals achieve self-sufficiency.

Qualification:
• Is a California licensed Registered Nurse, or a Licensed Vocational Nurse.
• Is CPR certified.
• Has two years experience in a hospital or clinic setting.
• Has experience working with those experiencing homelessness.
• Depending on the department assigned, a valid California driver’s license, a current DMV driving record, and proof of insurance may be required to fulfill job-related travel. Reasonable accommodations will be considered in accordance with applicable laws.
As the Nurse you will:
• Referral Coordination: In collaboration with the RCP Case Manager and RCP Supervisor, assess the appropriateness of hospital referrals. When suitable, interview patients at the hospital and coordinate with nursing staff and discharge workers to ensure a smooth discharge process.
• Participant Care Advocacy: Alongside the RCP Supervisor, guide and support case managers in coordinating participant care. This may include accompanying participants to medical appointments.
• Documentation Oversight: Supervise RCP staff to ensure accurate and timely charting of vitals and medication storage.
• Health Information Dissemination: Provide information on nutrition, physical activity, substance use, functional status at home, and other health and lifestyle issues.
• Medication Review and Advocacy: Review participants' current medications and advocate for continuity of treatment.
• Communication of Medical Needs: Interpret visit notes and orders from specialists and primary care providers, and communicate patients' needs to the RCP team.
• Health Monitoring: Address weight, eating habits, and exercise levels based on daily medical check-ins by the RCP team.
• Education and Counseling: Educate and counsel case managers on physician visits, helping clients understand and advocate for their health.
• Referral to Providers: Work with the RCP Case Manager to refer participants to physicians or appropriate providers when screenings indicate potential medical conditions.
• Detection and Reporting: Pay special attention to signs of depression, suicide risk, abuse, neglect, or cognitive changes, and report as required by law.
• Staff Training: Provide training to RCP staff on medical and medication issues relevant to clients in the program.
• Data Collection: Collect data as needed.
• Safety Compliance: Adhere to all safety rules and practices, ensuring maximum safety for oneself, fellow employees, clients, and children.
