About the Opportunity
Reporting to the Manager or designate, the interdisciplinary Clinical Leader provides leadership innovation and support to the clinical program area, to provide interdisciplinary collaborative person-centered care to patients. The interdisciplinary clinical leader works closely with the program leaders, physician leaders, professional practice coordinators, physicians, nurses, social workers, dietitians, and other team members to promote person-centered care to the patients of the One Door Chronic Disease Centre. Key duties include Developing and fostering healthy interdisciplinary team processes that develop a person-centered continuum of care, program planning and evaluation – ensuring practice is interdisciplinary and evidence-based, participating in the recruitment, selection, and orientation of new team members, supporting a quality approach to care delivery, coordinating and providing clinical care for patients.
Travel within Cumberland County will be necessary for this position.
About You
We would love to hear from you if you have the following:
- Bachelor's degree in a Health Care Program
- Registration with respective professional regulatory association if applicable.
- 3 Years of related experience in working with chronic disease patients,
- Prioritization and working with multidisciplinary teams of clinicians.
- Research skills;
- Experience in implementation of quality improvement program development and program evaluation initiatives
- Experience in Coaching and guiding problem-solving employee conflicts, in teams and groups
- Experience in setting agendas and chairing team meetings
- Team coordination skills;
- Patient navigation skills
- Effective communication
- Competencies in other languages an asset, French preferred

Hours
- Permanent Full time; 75 hours biweekly
- Shifts may include evenings ,weekends and holidays
Compensation and Benefits
$38.07 - $46.36 Hourly
