OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio’s first highly integrated care program for youth with complex behavioral health and multi-system needs. Care Management Entities (CMEs) are vital to the success of the OhioRISE model and will each serve a separate catchment area – part of a county or multiple counties that make up their geographic footprint.

Summary:
The Village Network is a subdelegate of the iamboundless OhioRISE CME, serving Licking, Knox, Delaware and Morrow counties. Applying the principles of Systems of Care, the OhioRise Moderate Care Coordinator is responsible for cultivating flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for OhioRISE members and in alignment with The Village Network’s Mission, Vision and Core Values. This role involves convening and facilitating Child and Family Teams (CFTs) developing individualized care plans, and ensuring collaboration among service providers, natural supports and community resources.

Essential Tasks, Duties, and Responsibilities:
Intake, Assessment and Planning: Initiate and maintain ongoing contact with children and families in the home and community setting; complete in-person assessments using assessment tools including the CANS; convene and facilitate in-person child and family team meetings; develop, implement and update the care plan; develop crisis safety plans; Request flex funds for services and items as approved by care plan; Provide in-person face to face and child and family team meetings at the required frequency and intensity required by tier level.
