Social Worker – Case Manager – Case Management
Choose to be an integral part of a team that helps patients achieve their greatest potential and work toward productive, independent lives. The Medical Master Social Worker is a professional member of the home health team who assists patient / family to cope with and to solve psycho-social and financial problems as they relate to medical problems; educates patient / caregiver regarding appropriate community resources. Participates in the coordination of care for assigned patients. PRN position covering DFW territory
Responsibilities
Using discretion and independent judgment, you, as the Licensed Social Worker, will assist patients and their families in coping with the unpredictability of illness within the context of the Home Health environment.
Other requirements of the Licensed Master Social Worker role include:
- Maintains a high level of Social Work expertise
- Works cooperatively as an ancillary interdisciplinary team member to identify and solve patient-specific and facility-wide needs and improve operations
- Demonstrates compassion while assessing individual patient or family social problems which impact their ability to interact with and fully utilize the health care system
- Demonstrates compliance with Home Health Care Management / Utilization Management, Discharge Planning, and Payor and Payor Referral Management policies and procedures specific to Home Health.
- Implement company standards of care, policies, and procedures
- Ensure all regulatory requirements are met
Qualifications

Other requirements of the Licensed Social Worker role include:
- Current state licensure as a Master Social Worker
- Master's Degree in Social Work required
- Licensed Master Social Worker (LMSW) required
- 1 year of previous experience required.
- Certified BLS completion required; for new hires
- Positive, caring and customer-service-oriented attitude
- Required Home Health or Hospice Experience
