DEPARTMENT SUMMARY:  The Transfer and Dispatch Center is based on the St. Charles Health Care Bend campus and includes several teams that are located together in a workspace that facilitates cross-team support and collaboration system-wide. These teams include the Transfer Center and Central Communications who are co-located with their partners in the Administrator on Duty. Additionally, this department facilitates several critical operations, communication, and facilities functions related to security, safety, and disaster preparedness.

POSITION OVERVIEW: The RN - Patient Logistics is responsible for oversight and coordination of patient acceptance, placement and movement (direct admissions, inter-system transfers, external hospital transfers, Emergency Department transfers, emergent, surgical, Interventional Radiology, Cath lab, and MDU cases) to an appropriate St. Charles Health System hospital in accordance with St. Charles Health System clinical standards and Joint Commission requirements, state regulations, and EMTALA and COBRA guidelines, ensuring patients meet requirements for acute care. Supports Transfer Center outgoing transfers as needed. Collaborates with physicians and department nursing leadership in order to centrally coordinate bed management, patient flow, with the goal of facilitating quality patient care in the most appropriate environment. Acts as liaison between community partners, providers, patient care departments and patient registration. Efficiently and effectively manage all hospital transfers in a timely manner, prioritizing transfers based on patient need and resources available at requesting facility.  Consult with on-call providers regarding patient care issues, needs and concerns and escalate as appropriate to leadership team and duty administrator. Supports dispatch services. This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Oversees dispatch operations and acts as a resource for support and problem solving.  Partners with Airlink Shift Manager as needed.

Responsible for overseeing coordination of direct admissions and hospital transfers.

Oversees coordination of inter-facility patient transports for transfers between St. Charles Health System facilities and collaborates with transfer / dispatch to ensure timely and appropriate patient transportation

Oversees facilitation of patient transfer requests, provider consultation requests, and intra-facility nurse to nurse reports using the transfer center recorded phone lines for quality management.  Maintains communication with all parties including but not limited to attending/accepting provider, charge nurse of receiving department, and referring facility.

Maintains oversight of transfers and prioritize according to acuity and patient need.  Consult with on-call providers and appropriate leadership regarding patient care issues, needs and concerns and escalate as appropriate. 

Communicate with provider and outlying facilities if placement of a specific patient will be problematic.  Use problem –solving skills to determine the best course of action, which will be appropriate plan of care and satisfactory to the patient, the provider, and referring facility. Assists referring facility in problem solving related to transfer care of patient.

Facilitates accurate and timely data collection, monitoring, evaluation, and analyzing of transfer problems for the purpose of developing and implementing process improvement, protocols, and policies. 

Excellent communication skills and the ability to succinctly exchange information with multiple entities.  Exhibits professional customer service skills including telephone and personal contact with all customers.

Establishes and maintains awareness of protocols, services and special programs at St. Charles Health System facilities.
