Facilitates entrance of new patients into care programs. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
This is a grant funded role. If in the future the grant funding is no longer available, this role may be eliminated.
The role is 40 hours weekly; Monday through Friday; 8:00 a.m.- 4:30 p.m.
A minimum of 2 years RN work experience is required.

Job Duties:
- Responsible for the coordination of pre and post hospital care.
- Serves as the liaison between the patient and caregivers.
- Coordinates appropriate multidisciplinary clinics.
- Performs duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the multi-disciplinary program.
- Facilitates entrance of new patients into care programs.
- Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
- Provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.
- Collaborates with other healthcare providers in coordinating and implementing the patient's plan of care including evaluation, treatment, follow-up and surveillance.
- Communicates with patient via mail, telephone, and in person regarding information pertinent to their care Coordinates patient and interdisciplinary activities to meet the needs of a defined group of patients.
- Acts as an effective resource.
- Coordinat's patient activities through the interdisciplinary clinics as appropriate.
- Reinforces patient and family education based on individual needs either in person or by telephone.
- Coordinates patient activities through available support services, such as support groups and social services as appropriate.
- Maintains systems and processes to assure timely follow-up and surveillance of post-follow-up patients.
- Provides nursing care and assists physician and procedures.
- Educates other team members in area of expertise.

Education:
Graduate from Specialty Training Program- (Required), Bachelor's Degree-Nursing (Preferred)Experience:
