Responsibilities
- Conduct comprehensive assessments of patients' physical conditions and develop individualized treatment plans
- Implement evidence-based physical therapy interventions to improve mobility, strength, and overall physical function
- Monitor patients' progress and modify treatment plans as necessary to achieve desired outcomes
- Educate patients and their families on proper self-care techniques and exercises to promote independence and prevent further injury
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and holistic patient care
- Maintain accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans
- Adhere to all regulatory and ethical standards in the provision of physical therapy services
Requirements
- Degree from an accredited Physical Therapy program
- Current Physical Therapist licensure in the state of Michigan
- A minimum of 1 year of experience in home health care is preferred, but not required
- Demonstrated knowledge of evidence-based physical therapy practices and techniques
- Strong interpersonal and communication skills to effectively interact with patients, families, and other healthcare professionals
- Ability to work independently and manage time effectively in a home care setting
- Proficiency in documentation and electronic medical record systems
