Home Health Physical Therapist (PRN)

Responsibilities

• Conduct comprehensive assessments of patients' physical conditions and develop individualized treatment plans
• Implement evidence-based physical therapy interventions to improve mobility, strength, and overall physical function
• Monitor patients' progress and modify treatment plans as necessary to achieve desired outcomes
• Educate patients and their families on proper self-care techniques and exercises to promote independence and prevent further injury
• Collaborate with interdisciplinary healthcare team members to ensure coordinated and holistic patient care
• Maintain accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans
• Adhere to all regulatory and ethical standards in the provision of physical therapy services

Requirements

• Degree from an accredited Physical Therapy program
• Current Physical Therapist licensure in the state of Michigan
• A minimum of 1 year of experience in home health care is preferred, but not required
• Demonstrated knowledge of evidence-based physical therapy practices and techniques
• Strong interpersonal and communication skills to effectively interact with patients, families, and other healthcare professionals
• Ability to work independently and manage time effectively in a home care setting
• Proficiency in documentation and electronic medical record systems
