Registered Dietitian

As a Strive outpatient Registered Dietitian, you will use your consultative expertise and compassion to help patients understand that what they eat directly affects their overall health and the progression of CKD. You will empower patients to take ownership of their health through building a relationship and working them through their goals. As a vital member of the patient's care team, you will educate patients and their families on a wide range of nutrition topics such as diabetes, hypertension and CHF to help slow the progression of CKD. This position is remote but may require some in-person work as well.   

Essential Functions

  • Consults with patients and their families on individualized Medical Nutrition Therapy (MNT) education and disease management, based on individualized patient needs including labs, nutrient requirements, knowledge and abilities, medical, economic, and social situation 
  • Assesses on-going nutritional status on subsequent follow-up visits to determine appropriate nutrition interventions and recommendations
  • Educates on meal planning, grocery shopping, and eating out, based on patient’s individualized goals 
  • Teaches patient about managing blood sugars, sodium/fluid restriction, limiting protein, and weight management.
  • Independently evaluates, develops and completes comprehensive nutrition assessment and reassessment consistent with best practices
  • Selects or develops education materials and methods appropriate for individual patients
  • Works to establish SMART goals and plan to achieve short- and long-term successes and improved quality of life for the patient 
  • Collaborates with Strive nurse practitioners, social work, pharmacy and physicians to communicate pertinent nutrition concerns
  • Develops strong patient/family relationships that fosters engagement and best outcomes  
  • Fosters a relationship with patients across their healthcare journey with a foundation of respect, honesty, empathy, no judgement and follow-through that promotes patient’s self-care and empowerment  
  • Skilled and passionate caring for patients with complex needs  
  • Drives prevention at every opportunity to slow progression of CKD by way of managing DM/CHF/weight, decrease exacerbations, and minimize complications while keeping in mind what the patient wants for their life   
  • Remains up-to-date and current on advances and trends in CKD treatment, guidelines, policies and procedures
