JOB SUMMARY
Provides safe, competent care for persons with wound, skin and ostomy needs. Assesses, plans, implements and evaluates treatment of wound, skin and ostomy patients in collaboration with the healthcare team . Facilitates education/training, planning, organization, and implementation within the hospital. Acts as a resource for hospital staff regarding wound/skin/ostomy care. Participates in performance improvement processes in order to ensure competent nursing care related to wound, skin, and ostomy care.
ESSENTIAL FUNCTIONS:
Assessment: Conducts a thorough, individualized assessment of the person with or at risk of wound/skin ostomy issues. Works with the patient, healthcare team and/or referring physician to identify treatment plans.
Analysis: Accurately and in a timely manner, analyzes the assessment data and establishes or modifies a treatment plan in collaboration with the medical team.
Planning: Accurately and in a timely manner, develops, maintains or modifies plan of care and communicates to the appropriate members of the health care team.
Implementation: Accurately and in a timely manner, implements and monitoring skin preservation strategies. Collaborates with other nurses and members of the health care team, delegating delegable nursing duties.
Evaluation: Evaluates the quality and outcomes of current interventions. Documents and communicates response to appropriate members to the healthcare team. Reassess patient response, revise and make changes in treatment in collaboration with medical team. Educates the patient, the patient’s family, and other support persons on ulcer prevention and treatment and ostomy care.
Education:
1. Provide ongoing education to hospital staff related to wound and skin care.
2. Assess employee competency on assigned units and develops competency assessment plans related to wounds, skin and ostomy care.
3. Participates in the development, implementation and evaluation for regional educations programs, orientation related to wound, skin and ostomy care.
4. Identifies needs for patient education. Facilitates the development and availability of patient educational material.
5. Provide quarterly Pressure Ulcer and Skin assessment for the hospital.
6. Conducts learning needs assessment to determine educational needs annually and as needed.
7. Actively participates in performance improvement.
