Job Summary:

The Pharmacy Intern embedded within the Mobile Access Pharmacy Program (MAPP) supports pharmacist-led clinical services, medication access initiatives, medication therapy management (MTM) services, operational workflows, program development activities, quality improvement initiatives, and community-based pharmacy services within a hybrid mobile health and population health care model.

The Pharmacy Intern will function as both a clinical support team member and operational/program support resource while gaining exposure to community-based clinical pharmacy services, medication access programs, population health initiatives, mobile healthcare delivery models, quality improvement, and public health-focused pharmacy practice.

Job Description:

Essential Duties and Responsibilities:

  • Clinical & Patient Care Support
    • Assist pharmacists with patient outreach, follow-up activities, medication access coordination, and continuity of care support activities.
    • Support patient scheduling, appointment coordination, and follow-up workflow management for pharmacist-led clinical services.
    • Assist with medication reconciliation, medication history collection, chart review activities, and preparation for Medication Therapy Management (MTM) encounters.
    • Identify patients who may benefit from pharmacist-led services through chart review, referral review, or Mobile Health Unit encounters.
    • Support patient education, medication-related outreach activities, and resource navigation activities under pharmacist supervision.
    • Assist with pre-visit planning, patient tracking activities, and preparation of pharmacist intervention opportunities.

  • Operational & Administrative Support
    • Assist with pharmacy-related workflow coordination, scheduling activities, phone tree coverage, and operational support tasks.
    • Support organization and maintenance of patient outreach monitoring, follow-up tracking systems, referral tracking activities, and scheduling workflows.
    • Assist with medication access activities including affordability resource identification, patient assistance program support, medication coordination activities, and pharmacy partner communications under pharmacist supervision.
    • Support coordination of medication-related supplies, inventory tracking activities, medication storage monitoring, and operational logistics associated with pharmacist-led services.
    • Assist with preparation and organization of materials, supplies, and documentation required for Mobile Health Unit community events and pharmacist-led services.
    • Participate in Mobile Health Unit operational support activities and designated community engagement events as needed.

  • Quality Improvement & Program Support
    • Assist with quality improvement initiatives, workflow optimization activities, implementation support projects, and operational program development activities.
    • Support data collection, data entry, reporting activities, and program evaluation efforts associated with pharmacist-led services.
    • Assist with research-related activities, literature reviews, project coordination, and dissemination efforts as appropriate.
    • Participate in development and maintenance of operational tracking tools, workflow resources, and quality monitoring processes.
    • Participate in interdisciplinary meetings, operational planning activities, workflow discussions, and program development initiatives as appropriate.
  • Documentation & Compliance
    • Complete assigned documentation, patient tracking activities, operational logs, and workflow-related tasks accurately and within established timeframes.
    •  Assist with preparation of pharmacist encounter materials, patient summaries, medication histories, and clinical documentation support activities under pharmacist supervision.
    • Maintain and update patient outreach records, scheduling trackers, referral tracking tools, MTM preparation materials, and other programmatic documentation systems.
    • Assist with data collection, data entry, quality improvement tracking, and program evaluation activities in accordance with established workflows.
