Description:

Inpatient Palliative Care Social Worker – Weill Cornell – Days

The Inpatient Palliative Care Social Worker is an integral member of the palliative care interdisciplinary team, providing compassionate support to patients and families facing chronic and life-limiting illnesses. This role includes offering supportive counseling, facilitating goals-of-care discussions, and collaborating on discharge planning and continuity of care. Additionally, the social worker is expected to take an active role in educating colleagues on palliative care and end-of-life best practices.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments as part of the Palliative care consult service for patients navigating chronic and/or life-limiting conditions.

  • Provide emotional support, grief counseling, and bereavement resources to patients and their families.

  • Actively participate in goals-of-care discussions, ensuring that patient and family wishes are supported.

  • Educate social workers, medical students, residents, and primary care teams on palliative care principles and best practices.

  • Advocate for interventions that alleviate patient and family burden and enhance coping strategies.

  • Collaborate with interdisciplinary teams to ensure seamless continuity of care and effective discharge planning.

Preferred Criteria:

  • Minimum two years of experience in palliative care or a related field.

Required Criteria

  • Master’s Degree in Social Work (MSW)

  • New York State License as a Licensed Masters Social Worker (LMSW) 

  • Experience in hospital case management and discharge planning.

  • Strong interpersonal and organizational skills, with the ability to participate in the management of complex cases.

  • Ability to work both independently and as part of a large interdisciplinary team.

  • Excellent written and verbal communication skills.
