Key Responsibilities:

Provide direct and supportive care to clients/patients, promoting their recovery and well-being.

Monitor and document client/patient behavior, vital signs, and medical symptoms.

Assist clients/patients with daily activities, such as personal care, medication administration, and transportation.

Facilitate group education/structured activities, promoting social interaction and skill-building.

Collaborate with the multidisciplinary team to develop and implement individualized treatment plans.

Maintain accurate records and reports, ensuring compliance with regulatory requirements.

Provide crisis intervention and support, as needed.

Education, Experience, and/or Credential Qualifications:

High School education or equivalent.

Two (2) years of responsible employment experience.

Experience in Human Services or a related field is preferred.

Additional Qualifications:

Must be at least 21 years old to operate a company-owned vehicle.

Valid driver's license in state of residency with an acceptable driving record.

Current auto insurance.

Ability to communicate effectively with clients/patients, families, and colleagues.

Basic keyboarding skills and ability to navigate electronic medical records.

Physical Requirements:

Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.

Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
