You will deliver hands-on, compassionate care directly in patients’ homes and through virtual platforms. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities.</p> <p>Your primary focus is delivering direct clinical care in the home: administering treatments, managing symptoms, conducting assessments, and being a trusted, present resource for patients and their families during one of the most challenging times of their lives. You will also support patients through virtual platforms and help them access Daymark’s services and support.</p> <p><strong>WHAT YOU'LL DO</strong></p> <p>In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs. In this role you will:&nbsp;</p> <ul> <li><strong>Deliver In-Home Clinical Care. </strong>Master hands-on care as a clinician in patients’ homes administering IV fluids, accessing ports, conducting point-of-care testing, and managing acute and chronic symptoms with confidence and skill.</li> <li><strong>Provide Urgent Clinical Support. </strong>Triage urgent needs from patients, escalations to a home visit or to our Nurse Practitioners.
