About the role

Description

Greater Nashua Mental Health is looking for a Forensic Case Manger to join our Adult Services Team! The Forensic Case Manager provides direct support, education, referral, and outreach to justice-involved adults diagnosed with severe mental health conditions, substance use disorders, and other co-occurring disorders. Supports individuals to access resources and services to meet basic needs through provision of case management services. Provides illness education and teaches recovery strategies through the provision of Enhanced Illness Management and Recovery, uniquely tailored to address criminogenic needs, aimed to divert individuals out of the legal system into long-term recovery. Serves as a liaison between treatment providers, legal professionals, and courts regarding recommendations for treatment and treatment participation. Facilitates empowering individuals to lead full and satisfying lives.  


Duties & Responsibilities: 

  • Works collaboratively with clients and providers to ensure connection to appropriate level of care and helps to navigate supports. 
  • Monitors multiple domains which impact mental health, including but not limited to housing, transportation, and vocational. 
  • Links and refers clients to needed resources outside of the mental health center, including but not limited to medical, educational, and social services. 
  • Works as part of an integrated team and communicates with colleagues, collateral contacts, supervisors, etc. regarding client care. 
  • Provides a combination of direct and indirect case management services. 
  • Provides individual and group Enhanced Illness Management and Recovery (EIMR) services. Maintain at least one active weekly EIMR group. Attend weekly EIMR group supervision. 
  • Completes documentation within 3 business days (with first day being date of service/completion). 
  • Serves as a court liaison sharing only pertinent information related to treatment plans, changes to plan, and participation. 
  • Ensures current Releases of Information are on file for any communication with 3rd parties, including legal professionals/court. 
  • Attend weekly Community Connections/Mental Health Court sessions as assigned. Ensure attendance to all required court hearings as assigned. 
  • Prepares for and attends all meetings as assigned by supervisor; meetings may include team meetings, community-based staff members' meeting, department meeting, individual supervision meetings, etc. 
  • Manages and respond to emails, voicemail, etc. in a timely and professional manner.
  • Performs other duties or special projects as required or as assigned. 

Requirements

Qualifications:   

  • Bachelor’s Degree in Psychology, Social Work, or similar field
  • 1-2 years of related work experience
  • Familiarity with mental health conditions and substance use disorders. 
  • Familiarity with community resources. 
  • Ability to teach effectively. 
  • Knowledge of Microsoft Office products (Email, Word, EXCEL).  
  • Strong collaborating skills, and an ability to work effectively both independently and as part of a team. 
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive. 
  • Excellent communication and interpersonal skills. 
  • Excellent organizational and time management skills. 
  • Ability to multitask and be flexible with regard to workload and assignments. 
  • Ability to exercise judgment and discretion.  
  • Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy
