Your Specific Responsibilities:
This position provides Ryan White Program medical case management to persons infected and affected by HIV/AIDS within the tri-county area of Polk, Hardee, and Highlands County.
- Maintain a caseload in excess of 150 patients.
- Perform program eligibility functions in accordance with Chapter 381, FS and Chapter 64D-4, FAC (the Eligibility Rule). Use determination guides set forth in the Florida HIV/AIDS Eligibility Procedures Manual.
- Responsible for the implementation and oversight of the care plan. Completion of tasks listed in the care plan include, but are not limited to, dental and mental health referrals.
- Assign clients’ acuity in coordination with the Case Manager Supervisor.
- The type and frequency of client contact is based on a client’s acuity. Case managers follow up to determine delivery of services.
- Completes comprehensive Needs Assessments of individuals presenting themselves for assistance through the agency.
- Responsible for the tracking and distribution of community-based vouchers; grocery vouchers; bus passes; and other resources distributed from the Ryan White Program.
- Enter, update, and upload, client documentation into Careware, HMS, Microsoft Access, and other required systems in a timely manner.
- Ensures proper documentation is maintained and submitted on all clients and program related activities. Enter detailed case notes on how a client’s needs are being addressed, how barriers to meeting these needs are being overcome, and how a client is progressing towards their care plan goals.
- Maintaining contact and communication with clients to prevent them from falling out of care.
- Facilitate linkage and re-engagement activities for clients who appear to be out of care.
- Participates in staff meetings and mandatory trainings.
- Actively engages in clinical quality management projects.
- Participates in technical staff conferences within the HIV unit.
Required Knowledge, Skills, and Abilities:
- Knowledge of theories and practices in counseling and social work.
- Knowledge of interviewing techniques.
- Ability to provide counseling and social work services to others.
- Ability to plan, organize and coordinate work assignments.
