The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

In a 24/7 environment, ensures patient flow, maintains critical access for specialty service, while balancing regional needs. Facilitates timely admission of patients to SMH. Using clinical judgment and experience, interacts collaboratively with physician/provider teams, Regional provider teams, at Strong Memorial Hospital, Affiliates, and Outside Hospitals, staff from the Emergency Department, Inpatient and Ambulatory areas, and Bed Management to ensure timely decision-making relative to disposition and care needs of people requiring inpatient placement. Facilitates timely transfer of patients from acute care hospitals to either Strong Memorial Hospital or Affiliate Hospitals based on assessment of patient needs and services provided by the various hospitals. Acts as a liaison between referring physicians and accepting physicians to facilitate communication regarding the smooth transfer of acute care patients. Triages appropriately to attending physician and service as required. Uses clinical judgment to ascertain level of care discrepancies, appropriately seeks direction from internal physician when balancing higher level of care is needs across the Affiliate Hospitals.

With great latitude and the expectation to exercise independent clinical judgement to support patient movement: The Transfer Center /Admitting Clinical Coordinator Registered Nurse (TC/ACC RN) provides direct support to Patient Flow throughout SMH and the Affiliate Hospitals. The TC/ACC RN is accountable for 24/7 patient flow. The TC/ACC RN participates in the management of patient triage, patient movement, mentoring house staff (RN and MD) and Regional MD's concerning patient transfers, the development and evaluation of census management strategies, and the monitoring of patient flow outcomes. The TC/ACC RN is accountable for supporting standards of care and providing consultation and expertise in the placement of specialty patients (Cardiology, Neurology, Transplant, Oncology, etc.) as well as managing placements of General Adult Services and Emergency Department patients. The TC/ACC RN is involved in activities at the service, institutional, and regional level, and contributes to goal achievement through personal action, effective communication and the facilitation of others. The TC/ACC RN participates in community and professional organizations, demonstrating commitment to the promotion of URMC visibility and future.

RESPONSIBILITIES:
Clinical Practice:

• Utilizes Clinical skills to Manage patient flow in an expert manner, following policies and procedures /recognizes trends or subtle changes inpatient /family response to care and responds effectively.
• Identifies complex patient problems and collaborates in developing a plan of care based on best practice standards and service availability; determines best patient placement based on expert clinical knowledge, current occupancy and staffing. Identifies patient safety issues and implements action plans or educational training based on needs. Demonstrates exemplary unit/service standards of documentation. Performs ongoing comprehensive assessment of regional, institution, department, service components that contribute to patient flow. Provides information to update practice standards and activities based on best evidence. Evaluates the effectiveness of patient placement based on clinical needs and revises as indicated.
