Description:

JOB DESCRIPTION SUMMARY: 

Jefferson County Public Health is currently hiring an Advance Practice Provider (APP) in our Sexual Health Clinic. This direct-care position involves clinical public health work to promote health of individuals and impact communicable disease rates and improve access to high quality medical care. JCPH provides sexual health services, including comprehensive contraceptive counseling and provision, including LARCS, STI/HIV/VH screening, testing, management, and prevention. There is potential to expand our scope of work in the future.   

This position is within the JCPH Clinic Services Division which provides immunizations, sexual health, and harm reduction services to our community. This position must be comfortable seeing gender diverse patients both in a clinic setting and conducting outreach with community partners, including homeless service centers and correctional facilitiesWorks as part of a team to deliver high-quality medical care. 

 

SCHEDULE: 

This position requires work to be performed at our onsite clinic location and at community/outreach clinics, including in the local county detention centerThis role is scheduled for 40 hours/week. A flexible schedule can be discussed with supervisor. 

 

COMPENSATION:  

Hiring Range: $102,000 – $117,000 USD Annually (1.0 FTE status) 

Compensation will be determined based on education, experience and skills.

ESSENTIAL DUTIES: 

  • Provide evidence-based, top-quality care to a diverse population.  

  • Adhere to the principles and mission of Jefferson County Public Health and federal, state, and foundation programming.   

  • Follow designated guidelines and division and program policies and protocols provision of care, including in assessment, diagnosing and treatment/management of medical conditions and preventive care. Consultation with attending providers as needed and required.   

  • Determines and assesses the health status of patients by reviewing records, taking comprehensive medical histories, and performing physical examinations.  

  • Initiates, performs, or directs other healthcare professionals to perform screenings and diagnostic tests.   

  • Monitors and evaluates medical condition of patient.  

  • Makes decisions regarding treatment changes and refers patient to specialized services as needed.  

  • Orders and administers appropriate medications 

  • Completes required EHR documentation thoroughly, and in a timely manner. Participates in clinical education of healthcare students as assigned.  

  • Involvement in clinical coordination
