Program: Enhanced Care Management

Status: Full-time, non-exempt

Hourly rate: $23.00 per hour

Location: SBCS Headquarters

Schedule: Monday through Friday: 8:30 a.m. – 5 p.m. Must be available to work until 7 p.m. two evenings per week to meet the various needs of the program.

Program:

The Enhanced Care Management (ECM) program provides person-centered care coordination to individuals with complex medical, behavioral health, and social needs. Services are delivered by a multidisciplinary team and include care coordination, referrals, advocacy, and linkage to medical, behavioral health, and community resources. The program emphasizes whole-person, culturally responsive, and trauma-informed care to improve health outcomes and member stability. 

 

RESPONSIBILITIES:

Under the supervision of the Program Director and Department Director, the Youth and Family Development Associate (YFDA), is responsible for providing case management and crisis intervention services to SBCS’ clients, families and the community, including in a school setting. YFDA also coordinates advocacy efforts for youth/families serviced within and across the system of care and assist youth/families in locating, coordinating and developing informal and formal resources and networks in the communities where they reside and/or attend school.

Services will be in keeping with agency goals, objectives, mission and style and provided within a team framework. YFDA is responsible for working with the team in developing team goals and objectives and in meeting those goals and objectives.

 

DUTIES:

  • Provide comprehensive case management services to vulnerable and high needs clients in person including at the office, in homes and in the community.
