About the role

Position Summary

Able to provide assistance to the Director of Case Management in the department, but not limited to, hiring/training/managing staff, schedule coordination, analysis and reporting, interfacing, collaborating and working with the other departments. Closely Conduct and Coordinate Case Management and Utilization Review Plan and related compliance policies and regulatory standards. Accountable for compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy. Coordinate Case Management and Social Services staffing, serve as a clinical leader, conduct performance and competency evaluation and maintain staffing records. Facilitate educational programs for Case Management Department on community resources use in discharge planning that encourages professional growth. Supervise and conduct the day-to-day activities of Case Managers, and Social Workers and Discharge Planners.

Minimum Education / Licensure / Certifications:

  • RN (BSN preferred). FL RN License
  • 2 years of acute hospital case management or healthcare experience 
  • Certification in Case Management preferred
  • Understanding of Federal Regulations related to discharge planning and Hospital utilization management; Excellent organizational skills; Verbal and written communication skills; Ability to coordinator activities of a diverse group; Demonstrated critical thinking and problem-solving skills; Computer literate in word, excel and PowerPoint.

Minimum Experience: Five (5) years relevant nursing experience

Physical Requirements: 

  • While performing the duties of this job, the employee is regularly required to sit, talk, and hear. 
  • The employee is frequently required to use fine motor skill (typing/data entry) and reach with hands and arms. 
  • The employee is frequently required to stand, walk and occasionally stoop, kneel or crawl. 
  • The employee must regularly lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.
