The Transfer Center Nurse in the Center for Transport Medicine is responsible for coordinating and facilitating the safe transfer of patients (neonatal, pediatric, adolescent, and adult) between facilities, ensuring a smooth and efficient process for all involved. This position requires excellent communication skills, critical thinking, and proficiency with information technology to effectively handle the complexities of patient care coordination while maintaining compliance with Commission on Accreditation of Medical Transport Systems (CAMTS) standards. 

Essential Job Functions:

  • Manages, prioritizes, triages, and processes patient (neonatal, pediatric, adolescent, and adult) transfers to and from Lurie Children’s.   
  • Documents clinical information (e.g., vital signs, physical exam findings, and laboratory/imaging results) to ensure appropriate care, transfer, and disposition. ��
  • Recommends appropriate medical interventions for the stabilization of critically ill patients to the referring facility based on transport team protocols, including anticipation of potential patient deterioration and emergency situations. 
  • Provides expert clinical guidance and care recommendations to referring facilities for all patients across the acuity spectrum to optimize patient outcomes and ensure appropriate level of care 
  • Maintains comprehensive documentation of intake call and treatment recommendations 
  • Serves as a central point of communication to ensure timely care, while also providing support and care recommendations to referring hospitals. 
  • Ensures clear, professional communication and exchange of clinical information between healthcare providers and services to facilitate seamless patient transfers.  
  • Collaborates with the House Supervisor to secure bed placement. 
  • Conducts triage and determines the appropriate transport team and mode of transport based on the patient's clinical condition and operational considerations.  
  • Provides exceptional customer service to all stakeholders, including patients, families, co-workers, referring center, and others to foster trust, loyalty, and a positive experience for everyone.  
  • Other job functions as assigned.

Knowledge, Skills and Abilities:

  • BSN graduate of a nationally accredited nursing program. 
  • Current licensure as a Registered Nurse in the State of Illinois active and in good standing. 
  • Minimum of 3 years pediatrics experience in high acuity inpatient setting (intensive care, emergency department, transport etc.). 
  • Demonstrates extensive knowledge of neonatal/pediatric diagnoses and management.  Equivalent experience may be considered.    
  • Bed management, admission & discharge and dispatch experience preferred. 
  • Basic Life Support (BLS) Certification required. Pediatric Advanced Life Support (PALS) preferred and required in the first year of hire.
  • Nurses are required to obtain an ANCC approved certification within one year of hire.  
  • Demonstrates strong leadership qualities with the ability to make independent decisions and take decisive action in high-stress situations.  
  • Excellent communication and interpersonal skills 
  • Critical thinking and problem-solving abilities 
  • Strong proficiency in Epic, Microsoft Excel, Word, and Outlook.

Bachelor's Degree: Nursing (Required)

Pay Range

$40.00-$66.00 Hourly
