**This position is 20 hours/week**

Who we need:

The Clinical Pharmacist will provide telephonic pharmacy care management services including medication regimen review and beneficiary education to optimize medication therapy in alignment with current treatment guidelines. This will involve close collaboration with nurse case managers, prescribers, and a multidisciplinary team to optimize health care and outcomes for beneficiaries. Chronic disease states include, but are not limited to: heart failure, COPD, diabetes, cardiovascular prevention, hypertension.

What you’ll do:

  • Perform telephonic medication evaluation consistent with standards of care for beneficiaries in states of licensure
  • Educate about medication therapy in a patient-friendly manner to beneficiaries and/or caregivers with varied levels of health literacy
  • Create and communicate concise, evidence-based recommendations to prescribers to optimize medication therapy
  • Document clearly and succinctly the medication evaluation and associated recommendations / action plan
  • Assist with program development and data analytics for case management, disease management, and pharmacy programs
  • Provide some oversight of clinical and non-clinical staff with responsibilities for training, work-flow management, case audits and evaluations
  • Represent pharmacy interests at multidisciplinary clinical case conferences, care or disease management program discussion, and client meetings as needed
  • Develop and present formal educational opportunities for case management staff and serves as an informal medication information resource
  • Other duties as assigned

What you’ll need:

Required Qualifications

  • Active, non-restricted pharmacist license in WV and willingness to obtain additional state licensure(s) to meet business needs
  • Multi-state licensure preferred (including KY, PA, OH, AL, IN, IL, or UT)
  • Doctor of Pharmacy (PharmD) preferred
  • Preferred qualifications include MTM certification, BCGP and/or BCACP

Knowledge, Skills, Abilities

  • Access to consistent internet for remote work
  • Strong grasp of current treatment guidelines and trends in healthcare
  • Excellent collaboration skills, a creative thinker with high energy and enthusiasm, and a positive team contributor
  • Ability to adapt to frequently changing work parameters
  • Strong written and oral communication skills

Experience

  • 3+ years of clinical experience or completion of residency program
  • Medication therapy management (MTM)
  • Preferred background in geriatrics, managed care/health plan
