ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Other duties and responsibilities may be assigned.
- Conducts an initial intake meeting within 24 hours of the client's arrival to the Sanctuary Program.
- Determines clients’ needs by completing intake interviews; and ensures clients are orientated to the program guidelines.
- Coordinate referrals for healthcare, mental health, and educational services.
- Enhances the program, by maintaining community relationships with all providers.
- Maintain and prepare clients’ records, by reviewing case notes.
Qualifications
REQUIRED EDUCATION
- High School Diploma / GED Equivalent
- Bachelor's degree in social work or human services field. preferred
- Bilingual in Spanish, preferred
REQUIRED EXPERIENCE
- Minimum two years in a behavioral health/substance use disorder treatment program
