Description

Position Function

The Clinical Case Manager-Treatment Services supports and serves youth in the custody of TDFPS and their families by providing and/or coordinating case management, medical and dental services, behavioral health supports and services,  family communication and supports/services, therapeutic services, crisis management and transitional services within a trauma-informed care framework and in compliance with all regulatory standards. The Clinical Case Manager-Treatment Services is responsible for the creation and maintenance of client files and ensuring all required case management services are delivered and documented in a timely manner. This includes developing assessments, developing service plans to meet the needs of clients, coordinating resources to stabilize behaviors, and working with the client and client’s family or managing conservator to secure a safe living environment upon leaving the Residential Treatment Center. This position does provide on-call support for clinical and program staff. 


Requirements:

Education: A Bachelor’s degree in social work, psychology, or related human services field from an accredited college or university is required. A Master’s degree and/or additional certification preferred.


Experience: 

With a bachelor’s degree, the position requires three years of experience that includes life skill/group counseling with youth, crisis intervention, intake and assessment, and/or case management. 

  • With a Master’s degree, the position requires one year of experience that includes counseling with youth, crisis intervention, intake and assessment, and/or case management. 
  • Experience  working with adolescents and families in crisis intervention, Trauma-Informed Care interventions, Trust Based Relational Intervention (TBRI) and strength-based case management techniques is strongly preferred
  • Excellent verbal and written communication skills
  • Working knowledge of basic computer skills

Other:

· Valid Texas driver’s license within 90 days of establishing Texas residency

· Acceptable driving record

  

Working Conditions:

The work environment includes possible exposure to children with behavior problems, including possible physical aggression. Exposure to illness and blood is also possible.

Maintain confidentiality and follow policies related to personnel records and client records.


Key Expectations/Responsibilities:

Program

  • Provide  expeditious and ongoing case management, family communication and support, aftercare and transition planning for Residential Treatment Center clients.
  • Coordinate services with TDFPS and/or OCOK caseworkers, families, community partners and program staff.
  • Assist with intake documentation and obtaining youth’s legal documents as needed.
  • Coordinate services to ensure compliance with 3 in 30 requirements for all youth and scheduling on-going medical/dental and behavioral health services.
  • Assist in effectively communicating with Residential Treatment Center staff and clinical team regarding the service plan goals and therapeutic needs and progress of each youth.
  • Assist in coordinating all service plan meetings.
  • Notify legal guardians of incidents in accordance with required timeframes and procedures. 
  • Participate and assist in facilitation of team staffing meetings.
  • Complete trainings and certifications as required by licensing, contractual, grant and agency standards.
  • Coordinate and support resident communication with family, including visits, phone calls and associated documentation. 
  • Assist in the scheduling and completion of resident appointments, groups, PALs and other activities in person or online.
  • Assist with other agency program’s activities as assigned.
  • Ensure that quality services are provided, and professional standards maintained.
  • Maintain a positive attitude of youth and family advocacy and assist in maintaining high staff morale.
  • Assist in the administration of medications, audits of medical documentation and management of med rooms. 
  • Assist in crisis management including de-escalation and intervention, debriefings, safety plans, on-call support, and triggered reviews as needed.
  • Maintain a positive work environment by fostering engagement through communication, feedback, collaboration, positive performance, and recognition.
