SUMMARY:
This position works directly with the Medical Director, Director of Nursing and Administrator to champion and promote evidence-based practice change efforts through management, development, implementation, and evaluation of education, programs, and policies to enhance the organization's ability to sustain person directed care and quality outcomes. This position is hourly/non-exempt and reports directly to the Director of Nursing (DON).
ESSENTIAL JOB FUNCTIONS:
- Manages skilled nursing facility readmission measure (SNFRM) while estimating risk-standardized rate of all-cause, unplanned hospital readmissions for SNF Medicare FFS beneficiaries within 30 days of discharge from the facility.
- Monitors, evaluates and manages care given to residents through assessing functional capabilities of residents and formulation of resident centered care plans.
- Manages person-centered care, evidence-based practice and culture change efforts by keeping abreast of the latest information/research and facilitating staff training in collaboration with the Director of Staff Development (DSD) and DON.
- Manages quality outcomes including SNF Value Based Purchasing program (VBP); ensures facility receives incentive payments for the quality of care given to Medicare beneficiaries.
- Ensures facility quality measures to achieve consistent performance above state benchmark.
- Participates in the QAPI process and maintains QAPI bulletin board per quality initiatives
- Analyzes SNF QRP submitted through MDS and Medicare Fee-For- Service Claims- CMS 5-star quality measures; develops performance improvement plans which do not meet facility/state benchmark; educates managers to including use of insights in PointClickCare.
- Responsible for Five-Star Quality Rating System outcomes, communicating changes to the interdisciplinary team
- Trains & educates staff on exceeding customer expectations through continuous Quality Assurance and Performance Improvement initiatives
- Responsible for clinical education in nurse training; periodically assesses competency for critical thinking, high-risk diagnosis/medications and advanced care planning; works closely with DON and DSD to implement annual nursing skills fair competency tests
- Collaborates with Medical Director, DON and Administrator annually to review, revise, update facility assessments
- Manages quality measures; performs daily rounds to gain opportunity for improvement
- Attends in-service; facilitates educational department & management meetings to implementing best practices to meet resident's needs.
- Assists with quality service and process initiatives, quality reporting and analytics
- Develops and provides support for process improvement teams; plans, implements and evaluates quality improvement activities
- Assists with Resident and Employee Satisfaction survey processes; participates on Employee Satisfaction Committee; supports efforts to boost employee training/morale
- Manages Policy and Procedure manuals; ensures completion/accuracy; facilitates timely review/approval of manuals.
- Works on special projects and committees assigned by the DON and Administrator.
- Fully complies with all provisions in the Corporate Compliance Policy and the Health Insurance Portability and Accountability Act (HIPAA) requirements
- Reports environmental or equipment repair needs to the maintenance department
Qualifications
QUALIFICATIONS:
- Active LVN or RN License
- Three-years minimum combined experience in skilled nursing or other related health care field; training and demonstrated knowledge of customer service and quality improvement programs essential.
- Familiar with health care facilities and services including state and federal regulations specifically Title XXll and COBRA; experience with elderly and the concepts of person-centered care and culture change.
- Efficient oral and written English communication skills; bilingual a plus
