The Virtual Nurse Care Coordinator provides clinical expertise, operational guidance and peer leadership for the virtual care program. This role serves as a central resource for staff, ensuring consistent application of protocols, promoting staff education, and advancing quality improvement initiatives to optimize patient outcomes. The coordinator supports day to day workflow, mentors nursing staff, facilitates communication across departments, and partners with leadership to expand system wide virtual care strategies.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided OR current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

2. Obtain certification in Basic Life Support within 30 days of hire date.

3. Bachelor’s of Science in Nursing (BSN).

EXPERIENCE:

1. Three (3) years of clinical experience.

1. One year experience within a virtual care setting.

1. Assist in development and implementation of unit specific staff educational activities within the scope of nursing practice.

2. Assist the department leadership in the assessment of new departmental employee learning needs and development of employee orientation and education plan to ensure consistency in implementation validation and documentation methodologies

3. Provides input and assists manager/director in annual review of unit Scope of Service, and performance improvement initiatives in conjunction with other departmental work teams

4. Assists departmental or unit teams in the development of annual competency analysis system requirements and criteria to assure knowledge, skills and attitudes that are necessary to fulfill role expectations in the institution.

5. Serves as a change agent in effecting appropriate changes within the health care delivery system as it relates to evidence based practice.

6. Contribute positively to the teamwork of the department by assisting other staff, contributing ideas and helping to resolve problems.

7. Serve as a resource and provide education to healthcare staff on identification, assessment, and management.

8. Medical Chart Review on data collection and patient’s individual needs. Utilizes clinical assessment skills and knowledge of EPIC or designated software application, to monitor patients effectively and efficiently in collaboration with the patient’s primary nurse. Utilize critical thinking skills and the specialized software to assess patients and preemptively address subtle signs and symptoms of clinical deterioration or status changes.

9. Defines expected outcomes that incorporate cost, clinical effectiveness, and are aligned with the outcomes identified by members if the inter-professional team.

10. In collaboration with department leadership, participates in data collection and review to develop performance improvement strategies

11. Partners with other members of health system to assist with system quality improvements.

12. Demonstrates leadership skills that emphasize ethical and critical decision-making, effective working relationships, and a systems perspective.
