Clinical Social Worker

Schedule: Tuesday, Thursday and Friday 8:30a-5p, with some on call responsibilities

Hours: 24 hours a week

Service Area: Louisville andsurrounding communities

Clinical Social Worker Position Summary:

As a member of the interdisciplinary team, provides social work services to patients and families as assigned. Services include: assessment, counseling, emotional support, assistance with financial and other social service needs, support and guidance regarding end of life decisions, legal affairs, and family assistance in planning for care giving.

Clinical Social Worker Position Qualifications:
• Must have Master’s degree in Social Work.
• CSW/LCSW license in Kentucky or Indiana required.
• Must have a minimum of two years clinical social work experience.
• One year social work experience in health care preferred.
• Must have a valid driver’s license in accordance to policy.
• Use of technology-based tools, including electronic medical record, strongly preferred.
