Scope:

As a Clinical Team Assistant, you will be a key connection between clients, referral sources, and care teams, helping individuals navigate their health needs while providing essential administrative support to keep Population Health operations running smoothly. Your role ensures seamless coordination of care, timely communication, and superior customer service—making a meaningful difference in the lives of those we serve.

As a Clinical Team Assistant, you will: 

  • Manage & Coordinate Referrals – Receive and process incoming population health referrals, assess client needs, determine eligibility, and initiate admission workflows.
  • Facilitate Communication & Scheduling – Serve as a primary contact for clients, caregivers, referral sources, and interdisciplinary teams, ensuring timely care coordination and scheduling of initial visits.
  • Support Electronic Health Records – Initiate client charts, document communications, and maintain accurate electronic health records for all referrals.
  • Provide Administrative & Office Support – Assist the Population Health team with report preparation, project coordination, meeting support, and office supply management.
  • Enhance Operational Efficiency – Answer calls, emails, and correspondence, handle data entry and office tasks, and contribute to population health strategy initiatives.

This job also requires: 

  • High school diploma or equivalent required; additional education in healthcare administration, public health, or a related field preferred.
  • Experience in healthcare, care coordination, or administrative support preferred.
  • Strong communication and customer service skills to interact professionally with clients, staff, and referral sources.
  • Proficiency in Microsoft Office and experience with electronic health records.
  • Strong attention to detail and organizational skills.
  • Ability to pass a state mandated background check.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with staff, referral sources, and clients.
