Job Description:

The Pathway of Hope Regional Coordinator provides implementation and oversight of the Pathway of Hope initiative in assigned Corps and service sites. The coordinator ensures consistency across locations regarding staff training, collection and entry of data, development of best practices and reporting on outcomes as well as the quality of services delivery across the division.    

What You Will Do:

Oversight of Pathway of Hope delivery at Corps and Service Sites

 

  • Provide administrative oversite for assigned Corps and service sites in the implementation of the Pathway of Hope Program
  • Provide direct case management consultation and technical assistance to case managers, case workers, care coordinators, and interns as assigned
  • Coordinate and facilitate regular onsite and/or virtual meetings with case workers, case managers, care coordinators, and interns as directed by Divisional Social Services Director
  • Meet with Corps and service site staff to offer support, information, and resources regarding community needs, opportunities, programmatic requirements (includes intakes and processes, intervention strategies, and collection of data to meet program reporting requirements)
  • Assist in recruiting, and interviewing of new employees directly involved in providing Emergency Assistance and Pathway of Hope as requested by Corps officers, Program Directors, and Divisional Social Services Director
  • Attend interagency meetings and participate in local collaboration as requested by caseworkers, case managers, care coordinators, Corps officers, Program Directors, and Divisional Social Services Director
  • Assist with special and seasonal events as requested, with the objective of identifying potential participants of the Pathway of Hope Program

 Implementation

  • Ensure quality of development and implementation with case workers, case managers and Corps officers and program Directors
  • Ensure consistency in program requirements by participating in regional and local calls with the Pathway of Hope Project Manager with Territorial Headquarters Social Services Department and other Regional Pathway of Hope Regional Coordinators throughout the Central Territory
  • Ensure relevance and consistency of Pathway of Hope Program by collaborating with the Territorial Project Manager and Divisional Social Services Director with development of policies and procedures, goals, and objectives with outcome-based measures
  • Ensure relevance of Pathway of Hope Program approach with Development and Finance Departments by remaining current on program funding resources, public and private, and support fundraising efforts as requested

 Program Evaluation and Outcome Management

  •  Ensure the accuracy of data entry into Service Point Database (SIMS) by reviewing entries on regular schedule
  • Oversee annual evaluation and outcome measurements for cluster of Corps and service sites to complete summary/review report for Territorial Project Manager and Divisional Social Services Director
  • Support bridging platforms of data collection into SIMS from external data platforms  
  • Report implementation challenges to the Territorial Project Manager and Divisional Social Services Director with recommendations for improvements
  • Perform quality assurance to ensure consistency of program data
  • Assist Divisional Social Services Director with the development of the annual divisional Pathway of Hope Plan as requested
  • Serve on committees related to case management, emergency assistance and crisis intervention, and building sufficiency as requested
  • Prepare reports regarding implementation and continuation of services across assigned clusters as requested by the Territorial Project Manager and/or Divisional Social Services Director<
