Overview
The Lifelong Health (LLH) Pharmacist provides clinical pharmacy support services to Riverside LLH facilities. Achieve maximum quality of service within the boundaries of expertise to all LLH customers. Responsible for knowing the pharmacy vendors' policies and procedures as it relates to Riverside LLH and when required acts as a liaison between the vendor pharmacy and Riverside LLH facilities. Perform all duties per Virginia state board regulations. Attend all facility meetings as it pertains to clinical/consulting pharmacy services. Provide oversight for pharmaceutical services as required. Participate in clinical/consulting pharmacy policy and procedure development as related to long term care facilities, and long term care facilities policy and procedure development related to pharmacy. Must maintain current knowledge of all federal and state drug laws, rules, and regulations as it relates to pharmacy services provided to Riverside LLH facilities. Must maintain professional decorum.

What you will do
  • Monitor patient profiles to assure appropriate dosage and therapy. Provide current drug information.
  • Monitor patient progress, reconcile data discrepancies, provide departmental updates, and promote services to Riverside LLH customers.
  • Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient.
  • Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals. This is accomplished through one on one meetings, general meetings, presentations, COR and quarterly meetings.
  • Participate in process improvement projects and other initiatives aimed at improving quality and cost of care

Qualifications

Education
  • Bachelors Degree, Pharmacy (Required) or
  • Doctoral Degree, Pharmacy (Preferred)

Experience
  • 1 year Gerontology or Long Term Care experience (Required)
  • 1 year Clinical pharmacy experience (Preferred)

Licenses and Certifications
  • Licensed Pharmacist - Virginia Department of Health Professions (VDHP) Upon Hire(Required)
  • Board of Pharmacy Specialties (BPS) - Board of Pharmacy Specialties Geriatric Pharmacy (Preferred)
