Guides patients through the healthcare system from screening through diagnosis and follow up, by assisting with access issues, developing relationships with service providers and tracking interventions and outcomes. Promotes a positive work environment which emphasizes respect to our customers, patients, patients’ families, providers, caregivers, and other departments. Serves as a clinical expert in the care of these patients.The Nurse Navigator will play a crucial role in our healthcare team, providing comprehensive care coordination and support to our patients. The successful candidate will be responsible for the following tasks:
1. Triage Valve/Watchman Phone Calls: Handle incoming calls related to valve/watchman, providing accurate and timely responses to patient inquiries.
2. Post-Procedure Follow-Up Calls: Conduct follow-up calls with patients after procedures to monitor their recovery and address any concerns.
3. Clinic Schedule and Patient Records: Manage the clinic schedule, assist in obtaining patient records, and ensure all follow-up needs related to patient records are met.
4. Patient STS Calculation: Calculate and monitor the Society of Thoracic Surgeons (STS) risk score for patients.
5. Incidental Findings Communication: Communicate incidental findings to patients and their Primary Care Physicians (PCPs).
6. KCCQ Documentation: Document Kansas City Cardiomyopathy Questionnaire (KCCQ) scores for patients.
7. Walk Tests: Conduct and monitor walk tests for patients as part of their ongoing care.
8. Cath/PSE Instructions: Provide instructions to patients for Catheterization Laboratory (Cath Lab) procedures and Patient Self-Exam (PSE).
9. Valve Clinic Support: Provide support to the Valve Clinic, assisting with various tasks as needed.
10. NICE Form for Watchman: Complete and manage the National Institute for Health and Care Excellence (NICE) forms for Watchman procedures.
11. Other: Other tasks as needed by service providers
