HOACNY provides cancer care services, Monday through Friday, day hours at three convenient locations in East Syracuse, Camillus and Auburn.  Thoracic/Pulmonary Nurse Navigator  - Registered Nurse (Monday Through Friday-no nights, weekends or holidays)

  

  1. Facilitate interaction with health care providers, and their staff including: primary care providers,  pulmonologist, thoracic surgeons, medical and radiation oncologists,  pathologists, and radiologists. 
  2. Initiate communication with  patients upon learning of a patient with a suspicious finding and  coordinate care with cancer specialists, medical oncologists, and radiation oncologists. 
  3. Identify personnel in various  departments of health care providers who are involved in patients with      Thoracic and Pulmonary health disparities and develop relationships with them; offer educational sessions to inform practitioners of Thoracic and      Pulmonary healthcare of the nurse navigator role, services provided.
  4. Guide patients through the health  care system.

  

  1. Work with community groups to increase cancer awareness surrounding pulmonary cancer and involve the community in      program planning if possible.
  2. Organize smoking cessation health community activities.
  3. Promote and accrue patients from the community to access our Lungs for Life screening program and Firefighter Screening      Event.
  4. Assist with the bi- monthly thoracic tumor conference,      maintaining statistics for patients presented at tumor board, including      the number of patients seen, physicians of records and other metrics as  determined by leadership.

Requirements

  

Basic Knowledge, Skills and Education Requirements

  • OCN certification preferred.
  • Registered nurse with 5 or      more years of experience in oncology or thoracic/pulmonary nursing.
  • Medical surgical nursing      experience. 
  • BSN preferred.
  • The cancer navigator must      have outstanding interpersonal, oral and written communication,      organizational and multi-tasking skills. The individual must poses knowledge of lung, esophageal, stomach,      anal and colorectal cancer, treatment modalities, cancer genetics      community resources and healthcare structure and function.
