Job Description:

An Advance Practice Registered Nurse (APRN) shall provide health care services under the general supervision of the Medical Director. The APRN shall function to assess, diagnose, treat, and monitor CNVH members when they have illness; to assess their functional strengths and deficits to facilitate the maintenance or rehabilitation of their abilities, or to assure needed intervention. To educate members, family, friends, community and CNVH staff in the prevention of illness and dysfunction. To enable members to grow and experience to the greatest extent possible with consideration of physical, mental, or emotional status. This practice shall mean health promotion, health supervision, illness prevention and diagnosis, treatment and management of common health problems, chronic conditions, and end of life care. The work schedule for this position is M-F, 8:00am-4:30 (with availability by phone for emergencies-after hours/holidays/weekends). $3/hr differential for hours worked.

Examples of Work and Job Expectations:

This position will perform, but will not be limited to, the following job duties in collaboration with the members’ physician/Medical Director.
-- Assess members’ health status through physical examination, ordering diagnostic tests (on a routine basis in collaboration with Medical Director), performing minimally invasive procedures, ordering more urgent lab tests when conditions warrant, therapeutic treatments, synthesizing and analyzing data, and applying advanced nursing principles. Updates medical records with his or her assessments in less than 72 hours.
-- Establish a comprehensive member database through recording of medical history, synthesis of medical records and recording of member assessments in the electronic medical record.

-- Formulate a diagnosis and/or identify functional or physiological deviations from normal, and provide education and management, refer when needed to an appropriate health care provider.

-- Practice evidence-based medicine and participate in QAPI and Risk Management discussions and join an Antibiotic Stewardship group if one is available across the State of Nebraska.

-- Under the framework of protocols and statutes, prescribe controlled substances listed in Schedule II, III, IV and V appropriate to the medical conditions of CNVH members.

-- Formulate, implement, and monitor a health management plan for each CNVH member assigned. Collaborate with nursing and pharmacy for effective management of the members as questions/concerns arise.

-- Provide education and counseling for members, family, CNVH staff on health promotion, health maintenance and health restoration. Availability to staff for questions/concerns that come across authorized means of technology. Supports the nursing staff (including with order entry if needed) to maintain effective professional relationships. Share information whether it be provider-to-provider information or information that arrives via the Clinic forms of communication with the appropriate healthcare team involved in the member care. Serve as a liaison to Hospice providers/nurses in a supportive role.
-- Initiate appropriate interventions in urgent and emergency situations until a physician is available. Address pain and symptom control with members when prescribed interventions are not effective.
-- Maintain a current, unencumbered APRN license
-- Maintain knowledge of current geriatric practices and competency in primary geriatric care


 

Requirements / Qualifications:

Minimum Qualifications: Successful completion of an accredited program for Registered Nurse and Nurse Practitioner.  Incumbents must have a current, valid license as a Registered Nurse and be certified as a Nurse Practitioner in the State of Nebraska

Preferred Qualifications: Prefer one-year experience in an advanced role in geriatric care and a Master or Doctorate degree in Nursing.

Knowledge of: the development and implementation of a plan of management and providing quality medical care; understand and interpret medical and diagnostic reports; symptoms of diseases; symptoms of medication reactions; therapeutic diets; physical, occupational and recreational therapies.

Skill in: counseling individual members and their families; interviewing members and document
