The population health team within Mount Sinai encompasses payer contracting, population health analytics and operations, care management, clinical integration, and population-based quality and efficiency management. The team includes 400+ employees with clinical, contracting, finance, IT, analytics, operations, and product development expertise.

Within MSHP and System Pharmacy, the pharmacy population health function drives value-based contract performance through medication optimization, utilization management, quality measure achievement, and care transition programs across a large and complex patient population. The Director, Population Health Pharmacy, will help provide strategic leadership for this portfolio, and will oversee all relevant operations, including clinical teams. . She/he will report to SVP and Chief Medical Officer of Population Health and will also work closely with Ambulatory Lead for System Pharmacy.

Mount Sinai Health Partners seeks a Director, Population Health Pharmacy who will:

(1) Help set and drive the population health pharmacy strategy across pharmacy quality performance, remote patient monitoring, utilization management and transitions of care. The Director translates organizational priorities in value-based care and total cost of care into a coherent, multi-year program roadmap and holds accountability for outcomes across the full portfolio. Important to this role will be:

  • Ability to connect clinical program performance to financial and quality outcomes at the enterprise level

  • Experience presenting pharmacy strategy and results to senior administrative and clinical leadership

  • Ability to make resource and investment decisions across competing program priorities

     

(2) Lead and develop pharmacy population health programs

Pharmacy Quality Programs

  • Oversee pharmacy quality initiatives including medication adherence, SUPD/ SPC and other pharmacy- related quality measures including day-to-day operations oversight, staffing, clinical operations, analytics and technology optimization and strategic planning

  • Prepare and oversee performance updates to senior leadership

  • Represent pharmacy in payer joint operating committees

     

Remote Patient Monitoring – Condition Management Program

  • Co-lead clinical design, strategy, and ongoing evaluation of condition management programs across the health system, with SVP/CMO of Population Health. 

  • Oversee program financial performance and operational KPIs, directing mitigation strategies as needed.  Develop and maintain relationships with key internal and external stakeholders to advance program goals and support clinical workflow integration.  Govern vendor relationships, technology infrastructure, and analytics tools to maintain efficient and scalable program operations.  Assess program impact on populations served, with emphasis on high-risk populations aligned to value-based care goals

Pharmacy Utilization Management

  • Oversee and drive equal-cost lower-efficacy initiatives

  • Oversee disease-state drug utilization programs including but not limited to PPIs, opioids, anti-VEGF agents, and antifungals

  • Oversee of site-of-care optimization strategy for high-risk ACO populations

  • In partnership with SVP/CMO, Population Health, support value-based care partnership arrangements with payer and VBC partners on pharmacy cost and quality

  • Possibly support benefit utilization management work and contribute to pharmacy benefit strategic initiatives for employee patients (i.e. PBM) 

  • Possibly implement and oversee pharmacy technology solutions including real-time-benefits tracker optimization
