About the role

Description

The Next Step Lead Case Manager (LCM) reports to the Manager of Housing Stability at 48th.?This position assists with overseeing the day-to-day responsibilities of case management of homeless families and single individuals in Denver programs.?This position requires a high level of multi-tasking assignments and facilitation to move participants towards long term stable housing. The primary role of this position is to provide additional support to the case management team, and as such should be proficient in case management tools such as 22-Things, Scribe, HMIS, and coordination with other teams. The Lead Case Manager position does not take calls or work outside of their assigned schedule.
 

Requirements

 General  

  • Manages a caseload as well as provides guidance to the case managers during working hours. The LCM will not take calls from staff or provide guidance outside of their assigned working hours. 
  • Fosters a culture of grace amongst staff that orients them around the mission of meeting individuals at their physical and spiritual points of need. 
  • Initiates and maintains good relations with all DRM departments. 
  • Maintains proficiency in DRM case management tools including 22-Things, SCRIBE, HMIS, and specific departmental tools, providing feedback to Homelessness Resolution Team (HRT) leadership on process and needs. 
  • Ensures all documentation is current with best practices and the implementation of changes has received the manager’s approval. 
  • Assists in facilitating a weekly case management meeting with all case managers and updates the Manager of Housing Stability with any outstanding challenges, concerns, and questions that need immediate attention. 
  • Collaborates with the Manager of Housing Stability in developing training and department manuals to equip case managers better and increase the department’s productivity. 
  • Exhibits creativity and adaptability to new situations to prepare case managers and lead them through a required program structure, process, and procedure change. 
  • Coaches all case managers on the case management process and procedure using the department’s approved manuals and resources. 
  • Provides SCRIBE or HMIS reports, spreadsheets, and statistics as requested. 
  • Demonstrates a professional and compassionate Christian example with staff, clients, and volunteers at the workplace. 
  • Oversees daily activities of case manager staff, interns, and volunteers. 
  • Reviews and audits case planning files to assist in case management coaching.  
  • Reviews performance data shared by the Manager of Housing Stability to monitor and measure the overall department goal and case manager’s performance. 
  • Attends assigned meetings.

  • Demonstrates respect, cultural competence, and knowledge of trauma-informed care in interactions with participants, staff, and other community providers. 
  • Upholds all residential and DRM program policies and written agreements. 
  • Develops a case plan with individualized goals based on individual needs, and facility/program requirements. Identifies action steps necessary to achieve goals, and monitors progress through scheduled follow up meetings. 
  • Performs comprehensive assessment of participant needs. 
  • Recommends the necessary level of intervention. 
  • Encourages and holds participants accountable to program guidelines. 
  • Monitors and assesses participant progress towards identified goals. 
  • Holds case planning meetings with clients, including scheduled and walk-in appointments. 
  • Works collaboratively to identify and assess escalating crisis situations and responds with de-escalation training strategies. 
  • Maintains professional best practices in areas of document storage, confidentiality and professional ethics when dealing with clients and client information. 
  • Oversees participant case files to ensure progress. 
  • Regularly updates all case planning records, services, and case notes in SCRIBE including uploading vital documents and ROIs. 
  • Facilitates community provider visits with clients including case note documentation in Scribe (DHS, MAT, Child Support, VA, SSI/SSDI Attorney). 
  • Collaborates, communicates, and advocates with service providers in the government, medical, mental health, housing, benefits acquisition, employment, legal, and veterans, etc. fields to aid clients toward a successful transition into long-term programs or sustainable housing. 
  • Facilitates community provider visits with clients as assigned, which includes signing up clients to meet with community providers. 
  • Collaborates with other DRM personnel regarding participant evaluation, goals and continuity of care.
